Main duties
1. To undertake a review of existing senior management roles and responsibilities within the Older People/Sensory and Physical Disability Services division. The review will focus upon existing capacity and functional arrangements and bring forward proposals that address current constraints and future opportunities.
2. With support from management colleagues lead on the review and development of proposals in relation specifically to the current operational arrangements within the sensory and physical disability social work team.
3. Work closely with colleagues from South West London and St Georges Mental Health Trust to prepare a project plan that will support the further integration of the two existing Older People Community Mental Health Teams.
4. Support the preparation of an initial project plan to explore opportunities for increased integration of the Short Term Assessment and Reablement Team with Intermediate Care currently operated through NHS Wandsworth Provider Services.
5. In support of the above projects prepare project plans and provide clear project management to taking them forward to the delivery of proposals which can then be presented for agreement and implementation.
